手术治疗对鼻腔大部或完全被堵塞，严重影响生理功能者，应先行手术治疗。术前应对病人进行全身检查，如患有 心血管疾病、哮喘发作期等，应待病情稳定后再行手术。手术方式主要有单纯鼻息肉切除术和鼻内筛窦切除术两种 。 1.单纯鼻息肉切除术用于根蒂清除，以前鼻息肉手术史者。在局麻下，以鼻息肉圈套器套住息肉要蒂部，勒紧后自鼻内向外用力急速拉出，使息肉连同根蒂一并摘除。 如有根蒂残留，可将其钳取干净。拉出息肉有时可使筛窦开放，此时若筛内粘膜已息肉样变，应同时行鼻内筛窦切 除术。 2.鼻内筛窦切除术主要适用于筛窦粘膜已为息肉组织所替代，有多次鼻息肉切除术史者。术中充分开放筛房，将 窦内息肉完全摘除。手术彻底可明显降低息肉复发率。近年来开展的鼻内窥镜鼻窦外科手术，为筛窦切除术提供了 一种精细、准确和有效的方法，使鼻息肉术后的复发率明显降低。文献报告可使复发率降到20%以 下。 总之，鼻息肉的治疗应参照下述原则： 1.鼻息肉治疗对初诊患者，如息肉较大，严重影响鼻功能，应先行外科治疗；息肉较小，鼻塞症状较轻者，可行内科治疗，1个 月后疗效不佳者再行手术治疗。 2.对伴有鼻窦感染者，宜先抗感染，或行鼻息肉摘除，以利开放鼻窦引流。
